Help with Tax form dividend income box 4 or 5?

I note that the Tax form has two boxes for dividend income:
  • TR3-4 - Dividends from UK Companies
  • TR3-5 - Other Dividends
My Tax Certificates have dividend income split into two sections:
  • UK Equity Dividend (UK Equities)
  • UK Unit Trust (Dividends Distributions From UK Authorised Unit Trusts And Open Ended Investment Companies )
Question is do both classes of dividend income add together and go into box 4 or should they be split seperately between box 4 and box 5, what does everyone else do?

    I separated mine but they just got added together in the calculation anyway

    Put dividends from individual stocks in box 4 and anything from funds goes into box 5. They are added together though.

    Would I be right in thinking then that Fundsmith is box 5?
