Life insurance policy without my knowlwdge

I found out yesterday that without my knowledge or consent my ex wife has taken out a life insurance policy on me. This was taken out 3 years after I left the marital home, and after we divorced. This is clearly forged and fraudulent. It is with a well known UK company.
What can I do in order to get the copies of the documents that we given, the information given, and how to ensure that I'm removed and the policy is stopped.
Also, is a subject access request something I can request as I'm horrified that this has happened.
As I said, I wasn't privvy to any of this, it's been done completely without my consent or knowledge. It was taken out in 2005.

It is possible to take out insurance on someone you have an insurability interest in. So if you were paying her maintenance that would stop on your death then that would seem a legitimate insurability interest.
if that is not the case then the insurance would be void and she will have wasted a lot of money.0 -
........ although, to be fair, the insured should be made aware of the policy in all cases.

I'm assuming it would be fraud if I had no knowledge of it. Would there not be a medical, consent,. Signatures requiredIt is hard to prove as you are looking at the period when no signatures were required. Medicals and consent would only be needed if the questions indicated risks that the insurer would want to look into further. A clean application with no signatures could be set up easily with no GP request or medical.
