WCA form

On the question of being able to go out i feel I should tick no as it says being able to so repeatedly etc.
Last month I went via taxi (couldn't walk there or use public transport) for an ecg and then the same to have second cover jab, after cancelling previous appointment because I couldn't bring myself to go out.
Would I be correct in ticking the no box?

    I also had taken diazapam the day before and on the day because of worsening panic attacks

    You answer based on the majority of your time.  So if you are mostly unable to leave your home, then the answer would be no.  But if there is space to explain why you have answered no, you should mention the issues you have faced.
    Suggest that you speak to your Doctors or any other medical professional that is helping you, if they are not aware of your daily battles.  Otherwise, if the assessment centre contacted your Doctors for more information, your Doctors would not be aware of you having problems leaving your home.
    The comments I post are personal opinion. Always refer to official information sources before relying on internet forums. If you have a problem with any organisation, enter into their official complaints process at the earliest opportunity, as sometimes complaints have to be started within a certain time frame.

    My mental health nurse knows and has said she will supply with me with a letter explaining my condition and the worsening of it.
    My gp knows of my condition but unless the nurse has told her, she won't know of the worsening so I'll get on to that.
    My mental health nurse is on leave and won't be able to do my letter until 7 June and my form has to be returned by the 11... if sent separately should I send letter to same address on form and do they read these things if not sent same time?
    Sorry for all the questions help very much appreciated.

    Evidence should ideally be sent with the form but if this isn’t possible then yes send it as soon as you can to the same address, which should be the health assessment providers. All my previous WCA forms have been sent straight to them and not to DWP.
    when filling out the forms you should add a couple of recent real world examples of what happened the last time you attempted that activity.

    Thank you Poppy, was at first tempted to chance returning the form a day or two late so I could include evidence but have read this would not be a good thing to do.
    Just hope they read it, I'm sure they will.

    Returning the WCA form late is not a good idea because you could be found fit for work this way. There's no extension  of time to return the form unless there's a mental health marker on your file, if there is there's some relaxation in the rules.

    If you will be sending in a letter after the WCA form has been returned then mention you will be doing this on the WCA form. Ideally take a copy of the letter and send in a copy and not the original. Also copy the UC50 / ESA50 form too if possible though in this instance you will need to send the original back to the health assessment provider.

    Definitely - even a photograph of each page would do, if you can't copy it or haven't typed the answers up on a computer.  Just so you have a record of what you wrote.

    Make sure you also put your name and NI number on everything you send.

    One day I may even get round to finding the cables and buy some ink for my printer..lol
