One of the questions is “Have you ever received a notice that affects your property” - do I need to declare a HMO licence renewal (one of the flats has been adapted to house 3 people) ?

Unless the wording of the question has changed, it's:
"In the past 3 years have you ever received a notice:
(a) advising that the owner of a neighbouring property has made a planning application?
(b) that affects your property in some other way?
(c) that requires you to do any maintenance, repairs or improvements to your property?"
from which I would say that (b) is going to include licensing applications by neighbours, so yes.1
