Right click on page, select 'Inspect Element', select '<div id="advBg_takeover" onclick="gotoLocation()"></div>' It's the seventh line, and then right click on this to select 'Delete node' from the menu and click on this option. Then you will be able to fill in the form.0

Ublock method works use the element picker and highlight the area it lists the same node. Alternatively add the below to your dashboard filters.
