Pension Contribution/Salary Sacrifice Limits

My understanding is that I can contribute upto £40k into a pension per annum, and unused allowance from previous two years can be added to that. I am nowhere near the lifetime limit.
Do employer contributions count towards that total, eg through salary sacrifice and/or pension matching say a 5% employee contribution?
Did those rumblings about salary sacrifice schemes becoming subject to BIK ever materialise?
Can I contribute to an employer pension scheme and a personal pension in the same tax year?
