Vodafone not paying back

First Post First Anniversary Combo Breaker
My wife recently discovered and extra £10 a month has been debited on her phone bill each month.
When enquiring to Vodafone, they discovered this had been occurring for the last 4 years! Basically billed for an additional phone number by mistake my wife has never actually used or been assigned a sim for.
They admitted their mistake and would get back to us. Despite many calls and promises each time to return our call, noone has called back, they refuse to give full names and instead each time a return call will be made.

Of course we expect this money to be refunded. Can anyone advise us where to go next, is this ombudsman next stop or? Feel very annoyed they are basically making no effort to respond.

    Follow the advice on the main Vodafone Complaints thread as always to contact the WRT - and tell her to check your bank statement and bills at least monthly and preferably daily (or don't be surprised when things like this happen).
