My partner was on SSP now moving to ESA

Hi everyone.
I am writing this on behalf of my partner. He is currently off work because on a herniated disc and is currently undergoing treatment
His sick pay ran out 3 weeks ago. He has been waiting for his SSP1 form since then. We have tried calling the employer nearly everyday. All they say is "we will call you back" and of course no one calls back and a range of excuses such as "the person who does it is ill" other times they just ignore him completely . His ESA claim can't be completed without the SSP1 form so he is kind of stuck in limbo.
We don't live together and I try to help him as much as I can but I can't always do it.
Is there anything he can do?

Your partner's employer should have completed this within 7 days of his not being entitled to sick pay.
Your partner can report the employer to HMRC (contact details in the link)
You can print off the form here:
https://www.gov.uk/government/uploads/system/uploads/attachment_data/file/423809/ssp1-print-may-2015.pdf
and send it to the employer with a stamp addressed envelope - you shouldn't have to do this but it may be the quickest way.
If there is a Head Office connected with the employer then email/write with a complaint about the employer.0 -

I have drafted up a cover letter for him to send to the head office.

To whom it may concern,
Please find enclosed in this letter a stamped and addressed envelope and an SSP1 form to be completed by you.
I am aware that this form should have been completed within 7 days of my SSP ending, I have now been waiting now for approximately 3 weeks. I understand that you maybe incredibly busy, so I have took it upon myself to provide you with the correct form to be completed and returned to me within 7 days of the date on the top of this letter.
If I do not hear from you within the stated time, I will be sure to pass on my concerns to HM Revenue and Customs which I am well within my rights to do so.
I believe I have given you fair warning and been patient long enough, despite now being in financial hardship.
