NHS Funding for Nursing home


Does anyone have any experience of the NHS funding nursing homes can you advise on how to start looking it? Do you need to apply or will the medical teams sort it out? I have done a bit of research and I know it's quite rare but is based upon medical needs. The person in questions is:-

Paraplegic and doubly incontinent from severed spinal cord
has chronic Kidney failure
has Oesteoporsis
has heart problems and a pacemaker
High BP
Chronic anaemia
Partially deaf
an is in their 90's.

They are in hospital at the minute but we've been told they'll need 24/7 nursing care so I'm trying to get my head around things. If the NHS funding is not applicable how do we sort out social care funding?
