ESA after 1yr new forms to fill in

Hi I was placed in ESA work related activity group after appealing ATOS decision last year. Since then my conditional has deteriorated and I wrote to ESA to inform them. Today I have rec'd a new form from ATOS Medical - has to be completed and returned before 4th August 2014 when my ESA ends. Im having trouble filling it in properly. Is there anywhere I can go to get help filling it in properly ?? I don't want to go to the Job Centre as the person their is so rude and ignorant. Please help need to get this form filled in asap
