I have not had a cashback deal with '3' but I do have one with another provider - The Carphone Warehouse.
I have recently successfully applied for my cashback with them, and I received it within 7 days.
My advice for anyone applying for a cashback is to send all the appropriate paperwork by registered mail. I am aware that this method is more expensive than normal first class post, but if the item is lost in the royal mail system then you are able to claim under their 'consequential loss' terms and conditions.
If the item successfully arrives at its destination then you have 'proof of delivery'. Proof of delivery is essential when trying to chase up a cashback cheque.
I realise that this post may not help the OP, but may help anyone else applying for a cashback from a mobile phone provider.

Are you sure it was with 3? check your original purchase receipts, as the cashback would likely be from a retailer, not 3 themselves. If 3 were to offer discounted line rental, it would come through on your bill directly, not some sort of cashback scheme or else a lot of upgraders here would be very interested.0 -
I had a cashback deal on a 3 phone through e2save.com and they were suprisingly quick and efficient at crediting the first cashback instalment which I have just claimed. I was expecting loads of aggro but it was real easy and they accepted a photocopy of the bill so I didn't need to send the original - I just sent it regular first class post and then phoned up a couple of days later to check they had received it, then got a confirmation text from them when they credited me.0
