Details needed to close bank account?

I've had a dormant account with Santander for a while now, and it keeps getting charged a £5/month fee. I did phone up to try and get it cancelled but they said it needs to be done in writing.
Has anyone got a list of info I'd need to put in?
Would a simple list of the sort code and account number be good enough or do you need to list other things like the card numbers etc?

Go into branch and close it. That is what I have just done.Estate Agent, Web Designer & All Round Geek!0

The specific account T&Cs will detail the correct closure procedure.
They won't close the account if you're overdrawn, or the system shows charges are to be applied to your account imminently.
They usually hold on to your request for 10 days or so before formally actioning it, just in case there are cheque and/or card transactions pending.
The Santander General Conditions state you must return the card and chequebook.0 -
If you have online banking, you can close the account via secure message. This is what I did.
