I feel a little bit bad about posting this having just read some other peoples real problems, but anyway...
In 2006 I ended my carphone warehouse O2 contract after it's 12 months expired.
Anticipating that my disconnection letter would be ignored, I stopped my direct debit.
At the time (not knowing any better) I did not send the letter by recorded delivery; however I don't think in this situation it makes it black & white as I can prove:
-That they have (recently) acknowledged my disconnection request by phone, and a later disconnection later being recieved.
-That the account was settled in full by the time my use of the service completely ceased.
They pursued the matter relentlessly for a few months until they finally closed the account, but demanded £120 for the few months after my disconnection request.
This debt got passed around a few debt collectors but I wasn't too worried. If I communicate with such companies at all it's usually along the lines of "Send two of them round and make sure they're big".
However, I recently bought an Equifax credit report and found some rather tarnishing information against my person raised by the Carphone Warehouse.
I disputed this with Equifax, who about 6 weeks later got back to me stating that Carphone Warehouse had informed them that the information is accurate and they are removing my notice of dispute.
So anyway, to get to the point; I need to take the dispute further, probably to the Carphone Warehouse themselves, but need a better idea of how. There are lots of template letters from USA sites but I don't think they're completely relevant to UK law.
So can anyone point me in the right direction of a template letter for this purpose?
And I don't suppose anyone could advise or suggest whether I have a case to claim compensation for the last few years of bad credit?

You should write to CW by letter, asking for action immediatley and for your file to be corrected. Then if they don't, either write to one of the Major newspapers consumer champoin, or to Oftel ie the Ombudsman.0

I also think you won't get anywhere either - probably should have dealt with it at the time, afterall you knew it existed but chose to ignore the letters - sorry
