Best place to get life insurance if you have a medical condition?

Hi
We're looking to get life insurance for myself and my partner. We are mid 30s and have two young children and a mortgage.
Thing is, I have a heart condition which is relatively minor (i.e no symptoms, see specialist every few years only), but obviously such things affect the premium.
It's such a minefield as I have to have a gp report done so each time I want a quote it's a rigmarole.
Can anyone recommend a reputable insurer?
