Travel Insurance: when to buy

A question I've thought about when I hear of people who have lost claims through exclusions.

Should you already have your insurance in place when you book your trip, flights, hotels etc.

If you already had insurance that's lapsed by the time you travel and take another policy with someone else for your trip who pays for any claims?
