Eye tests - complex presciptions

I need to get some new glasses soon as my old ones are the wrong prescription (last time I had them tested one of them had improved, so these are now too strong, but I didn't bother getting new ones as I only wear them indoors).

Anyway, I qualify for free tests due to having a complex prescription but I can't remember what to do - presumably I mention it when I ring up to book, and I have to fill in a form or something?

    You just sign a form when you get there, there isn't anything you have to do in advance.

    Thanks. I've been advised I should probably get the prescription and the glasses from the same place, so have to find some glasses I like the look of first!
