BT - did anyone else get offer for free evening & weekend calls and £15 vouchers?

Hi everyone
I received a mailing from BT back in November, offering me £15 worth of high street shopping vouchers if I took out a 12 month contract for free evening and weekend calls.

I signed up for this and sent the form back, but not received any shopping vouchers as yet.
I have rung BT, but no-one is able to help as I haven't kept the terms and conditions sent out in the mailing, re this offer.
I just wondered if anyone else had received this offer, and by miracle if anyone has kept the info - I'm hoping they might have done and hopefully there will be a phone number in the t&c's.
