Company car - not used for business - tax implications

A year or so back i got a company car from my work for my wife(i dont have one).
I am now being asked if i have used it for business or not as below. What would be the implications if i did or did not use it for business mileage

"We need to ensure that the secondary vehicle you have through the Scheme is not used by yourself for any business journeys, and that any business journeys you make are carried out in a privately owned vehicle.
If we do not hear from you this email will act as your declaration that you do not use your secondary scheme car for business mileage.
Therefore please let us know if this is not the case, and you do use the secondary scheme car for business mileage, so that we can ensure your personal tax liability is correct.

    Who is asking the questions? It's impossible to even guess at the implications unless we know who wants to know, i.e. your employer?, HMRC?, someone else?

    it is possible that the lease cost includes the insurance charge? if so then obviously business use would invalid an insurance policy covering only private use

    rather than you and us continuing to play guessing games here's a simple question: do you intend to lie or not?
    If not then just ask the lease company why they are asking!
