PPI on endowment policy?

I'm a bit confused by the pro forma reclaim letter as it looks like it's just for banks.
I had an endowment policy with Countrywide in 1998, cashing it in probably in 2012/2013. I have no paperwork connected with it now.
Would it have had PPI running alongside it? I know when I was not paying anything in to it, they were charging £200 a year to "run it"!
What is the letter I should send in the first instance to this company?

No. It is not possible for a PPI policy to be built into an endomwent policy.What is the letter I should send in the first instance to this company?
