Reapplying for Access to Work funding

I've been receiving Access to Work funding for nearly 3 years now. I was granted it in June 2014 and it expires at the end of this month.
It says on the paperwork that at the end of the time I have to reapply for further funding. Do they send me an application pack like with DLA/PIP - or do I just have to reapply myself ?
It was all organised for me 3 years ago by Remploy who worked with me at that time, but I'm not working with them now so wasnt sure what I should do ?

They just ring hubby up and do it over the phone. If you're concerned it's due phone them.0

I got a phone call from Wales when mine was due up. They wanted all new evidence so DLA letters and taxi quotes, they came to visit me at work and spoke to my manager. It was all a bit of a faff but that was in my old job. In this job I can get driven to work so I haven't bothered with A2W.0
