Hi I can only share our experience of Pisa and Tuscany - driving in Italy is something else. I am firmly of the opinion that Italians all think they are at Monza when behind the wheel, so be prepared for that. They certainly weren't terribly courteous on the motorway.
We actually used the rail network to travel between cities and avoid the urban roads.
As for car hire - I think we used Hertz without a problem, however try to note the experiences of others regarding handover and return issues also Pisa car hire queues can be lengthy at the weekend after flight arrivals.
Despite these issues have a great time in Tuscany - it is really beautiful part of Italy.

I think our last hire there was with Avis and it was a shuttle bus to and from the car hire sites. Watch out for the speed camera as you leave the airport ;)

When we went we got one online through either Holiday Autos or Auos abroad, can't remember now. They are brokers, and the actual contract was with Maggiore, which is Italy's big national car hire firm. It was fine, no problems at all. I think they are probably a bit cheaper than Avis or Hertz too. Just picked it up and returned it at the airport, no shuttle bus needed - it was 30 yards away!
When you go, make sure you go to Siena. If you go from Florence to Siena, don't take the motorway, take the old 'A' road (equivalent). Lots of lovely little towns/villages to have a look at. And stop at Greve and go to the best salumeria in the world!
